The success of the questionnaire method of collecting information depends largely on the proper designing of the questionnaire.
Designing questionnaire is a highly specialized job and requires a great deal of skill & experience..

While developing a questionnaire, the researcher has to be very clear on the following issues:

1. What information will be sought?
2. What types of questionnaire will be required?
3. How that (those) questionnaire(s) will be administered?
4. What the content of the individual question will be?
5. What the form of response of each question will be?
6. How many questions will be used and how the individual questions will be sequenced?
7. Whether the questionnaire shall be structured or unstructured?

Besides the earlier mentioned points, the following points may be kept in mind while designing a questionnaire:

1. Covering letter.
2. The number of questions should be as few as possible.
3. Questions of a sensitive nature should be avoided.
4. Instructions to the informants.
5. Footnotes
6. Questions should be capable of objective answer.
7. Answer to questions should not require calculations.
8. Pre-Testing the questionnaire.
9. Cross-Checks
10. Incentives to respondents.
11. Method of tabulation to be used.
12. Questions should be logically arranged.
13. Questions should be short and simple.

 For collecting primary data, to prepare a questionnaire is prerequisite. there are some rules should be followed so that data collecting process can smoothly. We some friends going to prepare a questionnaire to collect raw data from a particular rural area in Bangladesh. Thats why we are trying to succed the program.

Our research and analysis topic is like " the proportion of students to be dropped out or dropping out after HSC level" that we targeted. The objective of this analysis is to find out the core reason of those students,that Why the students are very reluctant?   The data will be collected from predetermined area.

As the subject is related to statistics, the above mentioned information and advice was really needed for us.
