Pharmacy / Rainy season and dengue fever
« on: September 10, 2015, 11:18:01 AM »
Dengue fever , also known as breakbone fever, is a mosquito-borne tropical disease caused by the dengue virus.Dengue fever is an infection caused by dengue viruses, of which there are four different serotypes known to infect humans.

Signs and symptoms

The disease has a sudden onset and symptoms may include:

1. Fever, as high as 106 F (41 C)
2. fever for 3 to 7 days
3. intense headache and pain behind the eyes
4. muscle and joint pain
5. loss of appetite
6. vomiting and diarrhoea
7. skin rash
8. bleeding, usually from the nose or gums.

Treatment

There is no specific antiviral treatment available.

General recommendations include controlling fever and pain with paracetamol rather than aspirin (aspirin may promote bleeding), and increasing fluid intake. Aspirin should not be given to children under 12 years of age unless specifically recommended by a doctor.

Pharmacy / All cholesterol is not bad
« on: September 07, 2015, 10:55:02 AM »
Cholesterol is oil-based and so does not mix with the blood, which is water-based. It is therefore carried around the body in the blood by lipoproteins. Lipoproteins, in order of molecular size, largest to smallest, are chylomicrons, very low-density lipoprotein (VLDL),intermediate-density lipoprotein (IDL), low-density lipoprotein (LDL), and HDL.

The parcels of cholesterol are carried by two types of lipoprotein:
   Low-density lipoprotein (LDL - cholesterol carried by this type is known as 'bad' cholesterol)
   High-density lipoprotein (HDL - cholesterol carried by this type is known as 'good' cholesterol).
HDL cholesterol absorbs cholesterol and carries it back to the liver, which flushes it from the body. HDL is known as “good” cholesterol because having high levels can reduce the risk for heart disease and stroke. Where LDL cholesterol makes up the majority of the body’s cholesterol. LDL is known as “bad” cholesterol because having high levels can lead to plaque buildup in your arteries and result in heart diseaseand stroke.

Cholesterol has four main functions, without which we could not live. It:
   Contributes to the structure of cell walls
   Makes up digestive bile acids in the intestine
   Allows the body to produce vitamin D
   Enables the body to make certain hormones.

What causes high cholesterol?
High cholesterol is a major risk factor for coronary heart disease, a cause of heart attacks, and reducing blood lipid levels lowers the cardiovascular risk.4
High levels of LDL lead to a build-up of cholesterol in the arteries, whereas HDL carries cholesterol to the liver for removal from the body.2 A build-up of cholesterol is part of the process that narrows arteries, called atherosclerosis, in which plaques form and cause restriction of blood flow.

Normal value of Cholesterol in blood serum is-
   <200 mg/dL (Normal)
             200-240 mg/dL (Borderline High)
                    >240 mg/dL (High)

Pharmacy / Signs and symptoms of dry eye with causes and prevention of it.
« on: September 02, 2015, 02:09:24 PM »
Dry eye syndrome
also known as keratoconjunctivitis sicca (KCS), keratitis sicca, sicca syndrome, xerophthalmia, or simply dry eyes, is an eye disease in which tear film evaporation is high or tear production is low.According to the National Health Service (NHS), UK, approximately between 17% and 30% of people have dry eyes at some time in their life.

Dry eye syndrome is affecting an increasing number of people worldwide.
What are the signs and symptoms of dry eye syndrome?

A patient with dry eye syndrome may have the following signs and symptoms:

a. Stringy mucus in or around the eyes
b. Eye sensitivity to smoke
c. Eye sensitivity to wind (eyes water more in the wind)
d. Redness of the eyes
e. Eye fatigue, even after reading for a relatively short period
f. Photophobia - sensitivity to light
g. Discomfort when wearing contact lenses
h. Tearing

What are the causes of dry eye syndrome?

Reduced tear production is also linked to:

1. Inflammation
2. Radiation
3. Diabetes
4. Rheumatoid arthritis
5. Vitamin A deficiency
6. Refractive eye surgeries

The following may also cause dry eyes:

a. Sun
b. Wind
c. High altitude
d. A very dry climate
e. Hot blowing air
f. Very dry air (airplane cabin air)
g. Working in front of a computer monitor (may slow down blinking rate)
h. Driving a vehicle (may slow down blinking rate)
i. Reading (may slow down blinking rate)
j. Smoke
k. Contact lenses
l. HIV

Prevention of dry eye syndrome

Keep the eyes clean - people with clean eyes are less likely to develop blepharitis. Use warm/hot (not boiling) water, dab some cotton wool into it, and gently clean your eyes. Massage your eye gently with a clean finger in a circular motion - this will help move the mucus from the eyelid glands.

Wind - wear wraparound glasses for protection from wind and hot air, which can cause dry eyes.

Computer monitor - the monitor should be just below eye level.

Avoid smoky places - if you smoke, give up. If you don't, avoid places where people smoke.

Humidifier - a humidifier in the home will help moisten the air, which will help reduce the risk of dry eyes. Some people say that spraying curtains with a fine spray of water also helps keep the air humid.

Nutrition - evidence is starting to appear indicating that a diet rich in omega-3 fatty acids may help prevent dry eye syndrome. Sources of omega-3 fats include salmon, trout, tuna, herring and sardines. If you are vegetarian/vegan, the following foods are rich in omega-3 fatty acids: canola oil, English walnuts, flax oil, ground flax seed, hemp oil, hemp seed, olive oil, pumpkin seeds, and soybeans.

Pharmacy / Correct micronutrient deficiencies
« on: December 07, 2014, 12:56:37 PM »
All severely malnourished children have vitamin and mineral deficiencies. As giving iron in acute phase can make infections worse, although anaemia is common, do NOT give iron initially but wait until the infection is controlled, child has a good appetite and starts gaining weight (usually by the second week)

Treatment

Give:

Vitamin A orally on Day 1 unless there is definite evidence that a dose has been given in the last month (for age >12 months, give
200,000 IU; for age 6-12 months, give 100,000 IU; for age 0-5 months, give 50,000 IU)

Pharmacy / Calcium Deficiency & Dietary sources
« on: December 04, 2014, 06:12:52 PM »
Calcium Deficiency
When calcium absorption is chronically low, because of low intakes, poor bioavailability, or conditions that impair intestinal absorption, there is a decrease in the serum ionized calcium concentration.
This in turn stimulates the release of PTH, which returns serum calcium to normal by increasing renal calcium reabsorption, stimulating the renal production of 1,25(OH)2D3, and inducing bone reabsorption.
The result of long-term calcium deficiency is accelerated bone loss in older individuals or the inability to fully achieve peak bone mass in younger individuals.
Dietary sources
The majority of dietary calcium in industrialized countries comes from milk products; one serving (i.e., 250 ml milk or yogurt or 40 g cheese) contains approximately 7.5 mmol (300 mg).
Nondairy sources (fruits, vegetables, and grain products) supply approximately 25% of total calcium. When substantial amounts of grains are consumed, for example, in breads or as maize products, these can be important sources, although the calcium in cereals tends to be less bioavailable than that in dairy products.
Other foods high in calcium include tofu set with a calcium salt, kale (cabbage), broccoli, and, increasingly, calcium-fortified juices and cereals.

Pharmacy / Iodine Deficiency Disorders andHow can be prevented?
« on: December 04, 2014, 06:08:55 PM »
Iodine Deficiency Disorders
The effects of iodine deficiency on the growth and development of a population that can be revented by correction of iodine deficiency, denoted by the term IDD, are evident at all stages, including particularly the fetus, the neonate, and in infancy, which are periods of rapid brain growth. The term goiter has been used for many years to describe the enlarged thyroid gland caused by iodine deficiency (Figure). Goiter is indeed the obvious and familiar feature of iodine deficiency, but knowledge of the effects of iodine deficiency on brain development has greatly expanded in the past 30 years
A hypothyroid cretin from Sinjiang, China, who is also deaf mute. This condition is completely preventable.
Correction of Iodine Deficiency
Iodized Salt
Since the successful introduction of iodized salt in Switzerland and the United States in the 1920s, successful programs have been reported from a number of countries, including those in Central and South America (e.g., Guatemala and Colombia) and Finland and Taiwan.
Universal salt iodization (USI) for India and subsequently for many other countries. This policy includes legislation to provide for compulsory iodization of all salt for human and animal consumption, and this legislation makes it illegal for noniodized salt to be available for human or animal consumption.
Potassium iodate is the preferred vehicle compared to potassium iodide because of its greater stability in the tropical environment. A dose of 20–40mg iodine as potassium iodate per kilo is recommended to cover losses to ensure an adequate household level. This assumes a salt intake of 10 g per day.
Iodized Oil
Iodized oil by injection or by mouth is singularly appropriate for isolated communities characteristic of mountainous endemic goiter areas. The striking regression of goiter following iodized oil administration, with improved well-being from correction of hypothyroidism, ensures general acceptance of the measure.
Iodized oil is more expensive than iodized salt but is used especially for severe iodine deficiency in remote areas. It provides instant correction of the deficiency and the consequent prevention of brain damage.
In a suitable area, the oil (1 ml contains 480 mg iodine) should be administered to all females up to the age of 40 years and all males up to the age of 20 years. A dose of 480 mg will provide coverage for 1 year by mouth and for 2 years by injection.
Iodized Milk
This is particularly important for infants receiving formula milk as an alternative to breast-feeding. An increase in levels from 5 to 10 mg/dl has been recommended for full-term infants and 20 mg/dl for premature infants. However, breast-fed infants will be iodine deficient if the mother is iodine deficient.
Iodized milk has been available in the United States, the United Kingdom and Northern Europe, Australia, and New Zealand as a result of the addition of iodophors as disinfectants by the dairy industry.

Coccinia grandis, the ivy gourd, also known as baby watermelon, little gourd, gentleman's toes, tindora or sometimes inaccurately identified as gherkin, is a tropical vine. It is also known as Cephalandra indica and Coccinia indica.
Coccinia indica (synonym Coccinia cordifolia), an herb growing abundantly in India, has been used in traditional treatment of diabetes. However, carefully controlled studies of its efficacy are lacking. This study aimed to evaluate the effectiveness of Coccinia cordifolia on blood glucose levels of incident type 2 diabetic patients requiring only dietary or lifestyle modifications.
In traditional medicine, fruits have been used to treat leprosy, fever, asthma, bronchitis and jaundice. The fruit possesses mast cell stabilizing, anti-anaphylactic and antihistaminic potential. In Bangladesh, the roots are used to treat osteoarthritis and joint pain. A paste made of leaves is applied to the skin to treat scabies.

Ivy gourd extracts and other forms of the plant can be purchased online and in health food stores. It is claimed that these products help regulate blood sugar levels. There is some research to support that compounds in the plant inhibit the enzyme glucose-6-phosphatase.Glucose-6-phosphatase is one of the key liver enzymes involved in regulating sugar metabolism. Therefore, ivy gourd is sometimes recommended for diabetic patients. Although these claims have not been supported, there currently is a fair amount of research focused on the medicinal properties of this plant focusing on its use as an antioxidant, anti-hypoglycemic agent, immune system modulator, etc. Some countries in Asia like Thailand prepare traditional tonic like drinks for medicinal purposes.
Ivy gourd is a plant. The leaves, root, and fruit are used to make medicine.

People take ivy gourd for diabetes, gonorrhea, and constipation.

Some people apply ivy gourd leaves directly to the skin for wounds.

Ivy gourd fruit and leaves are used as a vegetable in India and other Asian countries.

Pharmacy / Antimicrobial activity of Heliotropium indicum Linn (Hatir Shur)
« on: November 10, 2014, 01:11:29 AM »

Phytochemical screening of the extracts showed the presence of
Alkaloids, saponins, tannins, glycosides and flavonoids while phlobotamins, steroids, coumarin
and cardiac glycosides are absent in all the crude extracts. The antimicrobial screening of the
extracts showed significant zones of inhibition against 70% of the test organisms. The Minimum
Inhibition Concentration (MIC), Minimum Bactericidal Concentration (MBC) and Minimum
Fungicidal Concentration (MFC) against various organisms were conducted. The antimicrobial
analysis of the isolated components was also conducted. These prove the folkloric usage of the
plant and also indicated a broad spectrum activity of the plant extracts.
