In July, the American Red Cross declared an emergency blood shortage - it simply wasn't receiving enough donations to help all the patients that needed blood.

Now, researchers from the University of British Columbia may have found a way to address the problem, even if people aren't donating more: convert a less-usable blood type into one that anyone can receive.

They presented their research on Tuesday at a meeting of the American Chemical Society.

Blood types are different because of the sugars on the surface of the red blood cells the body creates. Type A has one type of sugar and Type B has another; Type AB has both sugars. Type O doesn't have any sugars.

If a person receives a blood transfusion of a blood type that's not their own, their immune system will attack and kill the donated blood cells.

For example, a person with Type A blood could never receive a Type B donation because their system would simply reject the new blood because the sugars aren't quite right.

Because Type O blood doesn't carry any sugars, anyone can receive it - it's the universally accepted blood type and, therefore, highly desirable.

In the past, researchers figured out that certain enzymes (molecules that cause chemical reactions) could remove the sugars from A, B, and AB blood cells, converting them into the more useful Type O.

However, as researcher Stephen Withers noted in a press release, they hadn't yet discovered an enzyme that was efficient, safe, and economical. Their search for that enzyme took them into the human gut.

Withers and his team already knew that the lining of the digestive tract contained the same sugars found on blood cells, and that bacterial enzymes within human feces stripped those sugars from the lining to power digestion.

Using this knowledge, the researchers were able to isolate an enzyme that strips the sugars from A and B blood types, transforming them into Type O 30 times more efficiently than any previously discovered enzyme.

For now the researchers are double-checking their findings. The next step would then be to test the enzyme in a clinical setting, which will help determine if the conversion process produces any unintended consequences.

All that extra testing could still take some time. But Withers is optimistic that his team's enzyme could be just the breakthrough we need to ensure anyone who needs a blood donation in the future will be able to receive one.

Researchers have identified a brand new 'micro-organ' inside the immune system of mice and humans – the first discovery of its kind for decades – and it could put scientists on the path to developing more effective vaccines in the future.

Vaccines are based on centuries of research showing that once the body has encountered a specific type of infection, it's better able to defend against it next time. And this new research suggests this new micro-organ could be key to how our body 'remembers' immunity.

The researchers from the Garvan Institute of Medical Research in Australia spotted thin, flat structures on top of the immune system's lymph nodes in mice, which they've dubbed "subcapsular proliferative foci" (or SPFs for short).

These SPFs appear to work like biological headquarters for planning a counter-attack to infection.

These SPFs only appear when the mice immune systems are fighting off infections that have been encountered before.

What's more, the researchers detected SPFs in human lymph nodes too, suggesting our bodies react in the same way.

"When you're fighting bacteria that can double in number every 20 to 30 minutes, every moment matters," says senior researcher Tri Phan. "To put it bluntly, if your immune system takes too long to assemble the tools to fight the infection, you die."

"This is why vaccines are so important. Vaccination trains the immune system, so that it can make antibodies very rapidly when an infection reappears. Until now we didn't know how and where this happened."

Traditional microscopy approaches analyse thin 2D slices of tissue, and the researchers think that's why SPFs haven't been spotted before – they themselves are very thin, and they only appear temporarily.

In this case the team made the equivalent of a 3D movie of the immune system in action, which revealed the collection of many different types of immune cell in these SPFs. The researchers describe them as a "one-stop shop" for fighting off remembered infections, and fighting them quickly.

Crucially, the collection of immune cells spotted by the researchers included Memory B type cells – cells which tell the immune system how to fight off a particular infection. Memory B cells then turn into plasma cells to produce antibodies and do the actual work of tackling the threat.

"It was exciting to see the memory B cells being activated and clustering in this new structure that had never been seen before," says one of the team, Imogen Moran.

"We could see them moving around, interacting with all these other immune cells and turning into plasma cells before our eyes."

According to the researchers, the positioning of the SPF structures on top of lymph nodes makes them perfectly positioned for fighting off infections – and fast.

They're strategically placed at points where bacteria would invade, and contain all the ingredients required to keep the infection at bay.

Now we know how the body does it, we might be able to improve vaccine techniques – vaccines currently focus on making memory B cells, but this study suggests the process could be made more efficient by also looking at how they transform into plasma cells through the inner workings of an SPF.

"So this is a structure that's been there all along, but no one's actually seen it yet, because they haven't had the right tools," says Phan.

"It's a remarkable reminder that there are still mysteries hidden within the body – even though we scientists have been looking at the body's tissues through the microscope for over 300 years."

সময়ের অভাব, নাস্তা বানিয়ে দেওয়ার মানুষ নেই, সকালে খেতে পারি না কিংবা ওজন কমানো ইত্যাদি বিভিন্ন অজুহাতে দিনের সবচাইতে গুরুত্বপূর্ণ খাবার ‘সকালের নাস্তা’ বাদ দিয়ে থাকলে নানান শারীরিক জটিলতায় পড়তে পারেন।

ডায়াবেটিসের ঝুঁকি: সকালের নাস্তা না খাওয়ার কারণে ডায়াবেটিস হওয়ার ঝুঁকি বাড়ে। হার্ভার্ড ইউনিভার্সিটির গবেষণা অনুযায়ী, যেসব নারী সকালে নাস্তা করেন না, তাদের টাইপ টু ডায়বেটিস হওয়ার আশঙ্কা বাড়ে ২০ শতাংশ। এর কারণ হল, সকালের নাস্তা বাদ দেওয়ার সঙ্গে শরীরের শর্করা সহ্যক্ষমতা কমে যাওয়া, প্রি-ডায়াবেটিস ও ডায়াবেটিস সম্পর্কযুক্ত।

ওজন বাড়ে: অনেকেই ওজন কমানোর জন্য একবেলা না খেয়ে থাকেন, আর বেশিরভাগ সময় সেই বেলাটা হয় সকাল। মজার বিষয় হল, এই সকালের নাস্তা বাদ দেওয়ার কারণেই তার ওজন কমে না, উল্টা বাড়ে। কারণ, সকালে না খেয়ে থাকার কারণে দুপুরবেলা ক্ষুধা বেশি লাগে। ফলে অতিরিক্ত খেয়ে ফেলে। আর একাধিক গবেষণায় প্রমাণিত হয়েছে, যারা সকালে নাস্তা খান, তারা স্বাস্থ্যকর ওজন বজায় রাখতে পারেন।

বিপাকীয় জটিলতা: সকালের নাস্তা না খাওয়ার কারণে বিপাক ক্রিয়ার হার কমে যায়। যে কোনো এক বেলার খাবার বাদ দিলে তা শরীরের স্বাভাবিক কর্মকাণ্ডের ওপর বিরূপ প্র্রভাব ফেলে। বিপাক ক্রিয়ার গতি কমানোর মাধ্যমে ক্যালরির ঘাটতি পূরণের চেষ্টা করে। তাই দীর্ঘ সময় না খেয়ে থাকলে তা শরীরের ক্যালরি পোড়ানোর ক্ষমতা কমাবে।

ক্ষুধার প্রভাব: দীর্ঘসময় না খেয়ে থাকালে অল্পতেই রেগে যাওয়া, মেজাজ গরম থাকা ইত্যাদি সমস্যা দেখা দেয়। গবেষণায় দেখা গেছে, যেসকল পুরুষ নিয়মিত সকালে নাস্তা খান, তাদের মন-মেজাজ থাকে ফুরফুরে ও ইতিবাচক। আবার সকালের নাস্তা না করলে শরীরের শর্করার মাত্রা কমে যেতে পারে। ফলাফল হতে পারে অকারণ বিরক্তি, অবসাদ কিংবা মাথাব্যথা।

মুখে দুর্গন্ধ: একবেলা না খেয়ে থাকা সামাজিক জীবনের জন্যেও ক্ষতিকর হতে পারে। সকালের নাস্তা না খাওয়া বা অনেক্ষণ না খেয়ে থাকলে মুখে বাজে গন্ধ হতে পারে। এর কারণ হল, খাওয়ার মাধ্যমে মুখে লালারস তৈরির প্রক্রিয়া উজ্জীবিত হয়, যা জিহ্বা থেকে ব্যাকটেরিয়া পরিষ্কার করে। না খেয়ে থাকলে এই প্রক্রিয়া বন্ধ হয়ে থাকে।

WHO’s first release of surveillance data on antibiotic resistance reveals high levels of resistance to a number of serious bacterial infections in both high- and low-income countries.

WHO’s new Global Antimicrobial Surveillance System (GLASS) reveals widespread occurrence of antibiotic resistance among 500 000 people with suspected bacterial infections across 22 countries.

The most commonly reported resistant bacteria were Escherichia coli, Klebsiella pneumoniae, Staphylococcus aureus, and Streptococcus pneumoniae, followed by Salmonella spp. The system does not include data on resistance of Mycobacterium tuberculosis, which causes tuberculosis (TB), as WHO has been tracking it since 1994 and providing annual updates in the Global tuberculosis report.

Among patients with suspected bloodstream infection, the proportion that had bacteria resistant to at least one of the most commonly used antibiotics ranged tremendously between different countries – from zero to 82%. Resistance to penicillin – the medicine used for decades worldwide to treat pneumonia – ranged from zero to 51% among reporting countries. And between 8% to 65% of E. coli associated with urinary tract infections presented resistance to ciprofloxacin, an antibiotic commonly used to treat this condition.

“The report confirms the serious situation of antibiotic resistance worldwide,” says Dr Marc Sprenger, director of WHO’s Antimicrobial Resistance Secretariat.

“Some of the world’s most common – and potentially most dangerous – infections are proving drug-resistant,” adds Sprenger. “And most worrying of all, pathogens don’t respect national borders. That’s why WHO is encouraging all countries to set up good surveillance systems for detecting drug resistance that can provide data to this global system.”

To date, 52 countries (25 high-income, 20 middle-income and 7 low-income countries) are enrolled in WHO’s Global Antimicrobial Surveillance System. For the first report, 40 countries provided information about their national surveillance systems and 22 countries also provided data on levels of antibiotic resistance.

“The report is a vital first step towards improving our understanding of the extent of antimicrobial resistance. Surveillance is in its infancy, but it is vital to develop it if we are to anticipate and tackle one of the biggest threats to global public health,” says Dr Carmem Pessoa-Silva, who coordinates the new surveillance system at WHO.

Data presented in this first GLASS report vary widely in quality and completeness. Some countries face major challenges in building their national surveillance systems, including a lack of personnel, funds and infrastructure.

However, WHO is supporting more countries to set up national antimicrobial resistance surveillance systems that can produce reliable, meaningful data. GLASS is helping to standardize the way that countries collect data and enable a more complete picture about antimicrobial resistance patterns and trends.

Solid drug resistance surveillance programmes in TB, HIV and malaria have been functioning for many years and have helped estimate disease burden, plan diagnostic and treatment services, monitor the effectiveness of control interventions, and design effective treatment regimens to address and prevent future resistance. GLASS is expected to perform a similar function for common bacterial pathogens.

The rollout of GLASS is already making a difference in many countries. For example, Kenya has enhanced the development of its national antimicrobial resistance system; Tunisia started to aggregate data on antimicrobial resistance at national level; the Republic of Korea completely revised its national surveillance system to align with the GLASS methodology, providing data of very high quality and completeness; and countries such as Afghanistan or Cambodia that face major structural challenges have enrolled in the system and are using the GLASS framework as an opportunity for strengthening their AMR surveillance capacities. In general, national participation in GLASS is seen as a sign of growing political commitment to support global efforts to control antimicrobial resistance.

#হিট স্ট্রোক কী?
গরমের সময়ের একটি মারাত্মক স্বাস্থ্যগত সমস্যার নাম হিট স্ট্রোক। চিকিৎসা শাস্ত্র অনুযায়ী, প্রচণ্ড গরম আবহাওয়ায় শরীরের তাপ নিয়ন্ত্রণ ক্ষমতা নষ্ট হয়ে শরীরের তাপমাত্রা ১০৫ ডিগ্রি ফারেনহাইট ছাড়িয়ে গেলে তাকে হিট স্ট্রোক বলে।
স্বাভাবিক অবস্থায় রক্ত দেহের তাপমাত্রা নিয়ন্ত্রণে ভূমিকা রাখে। কোনো কারণে শরীরের তাপমাত্রা বাড়তে থাকলে ত্বকের রক্তনালি প্রসারিত হয় এবং অতিরিক্ত তাপ পরিবেশে ছড়িয়ে দেয়। প্রয়োজনে ঘামের মাধ্যমেও শরীরের তাপ কমে যায়। কিন্তু প্রচণ্ড গরম ও আর্দ্র পরিবেশে বেশি সময় অবস্থান বা পরিশ্রম করলে তাপ নিয়ন্ত্রণ আর সম্ভব হয় না। এতে শরীরের তাপমাত্রা দ্রুত বিপৎসীমা ছাড়িয়ে যায় এবং হিট স্ট্রোক দেখা দেয়।

হিট স্ট্রোক কাদের বেশি হয়?
প্রচণ্ড গরমে ও আর্দ্রতায় যে কারও হিট স্ট্রোক হতে পারে। তবে কিছু কিছু ক্ষেত্রে হিট স্ট্রোকের সম্ভাবনা বেড়ে যায়। যেমন—
* শিশু ও বৃদ্ধদের তাপ নিয়ন্ত্রণ ক্ষমতা কম থাকায় হিট স্ট্রোকের সম্ভাবনা বেড়ে যায়। এ ছাড়া বয়স্ক ব্যক্তিরা যেহেতু প্রায়ই বিভিন্ন রোগে ভোগেন কিংবা নানা ওষুধ সেবন করেন, যা হিট স্ট্রোকের ঝুঁকি বাড়ায়।
* যাঁরা দিনের বেলায় প্রচণ্ড রোদে কায়িক পরিশ্রম করেন, তাঁদের হিট স্ট্রোকের ঝুঁকি বেশি। যেমন কৃষক, শ্রমিক, রিকশাচালক।
* শরীরে পানিস্বল্পতা হলে হিট স্ট্রোকের ঝুঁকি বাড়ে।
* কিছু কিছু ওষুধ হিট স্ট্রোকের ঝুঁকি বাড়ায় বিশেষ করে প্রস্রাব বাড়ানোর ওষুধ, বিষণ্নতার ওষুধ, মানসিক রোগের ওষুধ ইত্যাদি।

হিট স্ট্রোকের লক্ষণগুলো কী?
তাপমাত্রা বাড়ার সঙ্গে সঙ্গে দেহে নানা রকম প্রতিক্রিয়া দেখা দেয়। প্রাথমিকভাবে হিট স্ট্রোকের আগে অপেক্ষাকৃত কম মারাত্মক হিট ক্র্যাম্প অথবা হিট এক্সহসশন হতে পারে। হিট ক্র্যাম্পে শরীরের মাংসপেশিতে ব্যথা হয়, শরীর দুর্বল লাগে এবং প্রচণ্ড পিপাসা পায়। এর পরের ধাপে হিট এক্সহসশনে দ্রুত শ্বাসপ্রশ্বাস, মাথাব্যথা, ঝিমঝিম করা, বমিভাব, অসংলগ্ন আচরণ ইত্যাদি দেখা দেয়। এই দুই ক্ষেত্রেই শরীরের তাপ নিয়ন্ত্রণ ঠিক থাকে এবং শরীর অত্যন্ত ঘামতে থাকে। এ অবস্থায় দ্রুত ব্যবস্থা না নেওয়া হলে হিট স্ট্রোক হতে পারে। এর লক্ষণ গুলো হলো—
* শরীরের তাপমাত্রা দ্রুত ১০৫ ডিগ্রিº ফারেনহাইট ছাড়িয়ে যায়।
* ঘাম বন্ধ হয়ে যায়।
* ত্বক শুষ্ক ও লালচে হয়ে যায়।
* নিশ্বাস দ্রুত হয়।
* নাড়ির স্পন্দন ক্ষীণ ও দ্রুত হয়।
* রক্তচাপ কমে যায়।
* খিঁচুনি, মাথা ঝিমঝিম করা, অস্বাভাবিক আচরণ, হ্যালুসিনেশন, অসংলগ্নতা ইত্যাদি।
* প্রস্রাবের পরিমাণ কমে যায়।
* রোগী শকেও চলে যায়। এমনকি অজ্ঞান হয়ে যেতে পারে।

প্রতিরোধের উপায় কী?
গরমের দিনে কিছু সতর্কতা মেনে চললে হিট স্ট্রোকের বিপদ থেকে বেঁচে থাকা যায়। এগুলো হলো—
* হালকা, ঢিলেঢালা পোশাক পরুন। কাপড় সাদা বা হালকা রঙের হতে হবে। সুতি কাপড় হলে ভালো।
* যথাসম্ভব ঘরের ভেতরে বা ছায়াযুক্ত স্থানে থাকুন।
* বাইরে যেতে হলে মাথার জন্য চওড়া কিনারাযুক্ত টুপি, ক্যাপ বা ছাতা ব্যবহার করুন।
* বাইরে যাঁরা কাজকর্মে নিয়োজিত থাকেন, তাঁরা মাথায় ছাতা বা মাথা ঢাকার জন্য কাপড়জাতীয় কিছু ব্যবহার করতে পারেন।
* প্রচুর পানি ও অন্যান্য তরল পান করুন। মনে রাখবেন, গরমে ঘামের সঙ্গে পানি ও লবণ দুই-ই বের হয়ে যায়। তাই পানির সঙ্গে সঙ্গে লবণযুক্ত পানীয় যেমন-খাবার স্যালাইন, ফলের রস, লাচ্ছি ইত্যাদিও পান করতে হবে। পানি অবশ্যই বিশুদ্ধ হতে হবে।
* তাপমাত্রা বৃদ্ধিকারী পানীয় যেমন-চা ও কফি যথাসম্ভব কম পান করা উচিত।
* রোদের মধ্যে শ্রমসাধ্য কাজ করা থেকে বিরত থাকুন। এসব কাজ সম্ভব হলে রাতে বা খুব সকালে করুন। যদি দিনে করতেই হয়, তবে কিছুক্ষণ পরপর বিশ্রাম নিতে হবে ও প্রচুর পানি ও স্যালাইন পান করতে হবে।

আক্রান্ত হলে কী করণীয়?
প্রাথমিকভাবে হিট স্ট্রোকের আগে যখন হিট ক্র্যাম্প বা হিট এক্সহসশন দেখা দেয়, তখনই প্রয়োজনীয় ব্যবস্থা নিলে হিট স্ট্রোক প্রতিরোধ সম্ভব। এ ক্ষেত্রে ব্যক্তি নিজেই যা করতে পারেন তা হলো—
* দ্রুত শীতল কোনো স্থানে চলে যান। যদি সম্ভব হয়, ফ্যান বা এসি ছেড়ে দিন।
* ভেজা কাপড় দিয়ে শরীর মুছে ফেলুন। সম্ভব হলে গোসল করুন।
* প্রচুর পানি ও খাবার স্যালাইন পান করুন। চা বা কফি পান করবেন না।

কিন্তু যদি হিট স্ট্রোক হয়েই যায়, তবে রোগীকে অবশ্যই দ্রুত হাসপাতালে নিতে হবে, ঘরে চিকিৎসা করার কোনো সুযোগ নেই। এ ক্ষেত্রে রোগীর আশপাশে যাঁরা থাকবেন তাঁদের করণীয় হলো—
* রোগীকে দ্রুত শীতল স্থানে নিয়ে যান।
* তাঁর কাপড় খুলে দিন।
* শরীর পানিতে ভিজিয়ে দিয়ে বাতাস করুন। এভাবে তাপমাত্রা কমাতে থাকুন।
* সম্ভব হলে কাঁধে, বগলে ও কুচকিতে বরফ দিন।
