পবিত্র রমজান মাসে একজন ডায়াবেটিক রোগীর খাদ্যাভ্যাস, ব্যায়াম, জীবনাচরণ ও ওষুধের নিয়মে বড় ধরনের পরিবর্তন ঘটে। এক মাসের জন্য এই নতুন ধরনের জীবনাচরণপদ্ধতির সঙ্গে খাপ খাইয়ে নিতে তাঁদের কিছু নিয়মকানুন পালন করতে হয়। আন্তর্জাতিক ডায়াবেটিস ফেডারেশন (আইডিএফ) রমজান মাসে ডায়াবেটিসে আক্রান্ত রোগীদের স্বাস্থ্যকর জীবনযাপন ও চিকিৎসাপদ্ধতি নিয়ে গাইডলাইন প্রকাশ করেছে। এরই আলোকে বাংলাদেশের রোজাদার ডায়াবেটিক রোগীদের জন্য সংক্ষেপিত আকারে এই নির্দেশনামা তৈরি করা হয়েছে।

রোজায় ডায়াবেটিক রোগী

কিছুসংখ্যক জটিল রোগী বাদে অধিকাংশ ডায়াবেটিক রোগী বড় ধরনের কোনো সমস্যা ছাড়াই রোজা পালন করতে পারেন। রোজা পালনের কারণে ডায়াবেটিক রোগীরা কিছু স্বাস্থ্যঝুঁকির সম্মুখীন হতে পারেন। যেমন ১. হাইপোগ্লাইসেমিয়া বা রক্তে শর্করা স্বল্পতা, ২. হাইপারগ্লাইসেমিয়া বা রক্তে শর্করা অত্যধিক বেড়ে যাওয়া, ৩. ডিহাইড্রেশন বা পানিশূন্যতা, ৪. কিটো অ্যাসিডোসিস এবং ৫. থ্রম্বোসিস বা রক্ত জমাট বাঁধা।

ঝুঁকি বিবেচনা করে কিছু রোগীকে চিকিৎসকেরা উচ্চ ঝুঁকিসম্পন্ন হিসেবে চিহ্নিত করেছেন, যাঁদের রোজা পালন না করাই ভালো, তাঁরা হলেন, রোজার আগে তিন মাসের মধ্যে যাঁদের তীব্র হাইপোগ্লাইসেমিয়া বা হাইপারগ্লাইসেমিয়াজনিত জটিলতা হয়েছে, যাঁদের বারবার রক্তে শর্করা কমে যাওয়ার প্রবণতা রয়েছে বা যাঁরা রক্তে শর্করা কমে গেলে টের পান না, ডায়াবেটিসজনিত কিডনি রোগ বা হৃদ্‌রোগ আছে যাঁদের, রমজানরে সময় আকস্মিক অসুস্থতা (জ্বর, সংক্রমণ ইত্যাদি), বয়স্ক রোগী ও অন্তঃসত্ত্বা ডায়াবেটিকে রোগী যাঁরা ইনসুলিন গ্রহণ করেন।

রমজানে খাদ্যাভ্যাস ও ব্যায়াম

রমজান মাসে সাধারণত ইফতার ও সাহ্‌রিতে দুটি মূল খাবার গ্রহণ করা হয়। লক্ষ রাখতে হবে যে রমজান মাসেও দৈনিক ক্যালরি চাহিদা একই রকম থাকবে, কেবল সময়সূচিতে পরিবর্তন আসতে পারে। অন্য সময়ের মতোই চিনি বা চিনিযুক্ত খাবার খাওয়া নিষেধ। জটিল শর্করা যেমন: লাল চালের ভাত, রুটি, ওটস, কর্নফ্লেক্স খাওয়া ভালো। যথেষ্ট পরিমাণে শাকসবজি, তাজা ফলমূল খেতে হবে। ইফতারে অতিরিক্ত ভাজা-পোড়া ও তেলযুক্ত উচ্চ ক্যালরি খাবার পরিহার করা উচিত। একসঙ্গে অনেক খাবার না খেয়ে ভাগ করে খাওয়া উচিত। রমজান মাসে প্রযোজ্য দুটি খাদ্যতালিকা এখানে সংযুক্ত করা হলো। এ ছাড়া কয়েকটি বিষয়ের দিকে লক্ষ রাখুন।

* সাহ্‌রি না খেয়ে রোজা রাখবেন না। সাহ্‌রিতে ভাত বা রুটি, সবজি, মাছ বা মাংসসহ একটি পূর্ণাঙ্গ খাবার খেতে হবে।

* ইফতারে শরবত বা মিষ্টি জুস না খেয়ে ডাবের পানি, ফলের রস, লেবুপানি পান করতে পারেন।

* ইফতার ও সাহ্‌রির মাঝখানে দুধ, ফলমূল, চিড়া, দই ইত্যাদি খাওয়া যেতে পারে।

* রাতের বেলা পর্যাপ্ত পানি পান করুন।

রমজান মাসে দিনের বেলায় ব্যায়াম বা ব্যায়ামের উদ্দেশ্যে হাঁটাহাঁটি না করাই ভালো। সন্ধ্যার পর হালকা ব্যায়াম করা যেতে পারে।

রমজানে রক্তে সুগার পরিমাপ করুন

রমজান মাসেও সময় নির্ধারণ করে রক্তে শর্করা পরিমাপ করতে হবে। খারাপ লাগলে বা হাইপোগ্লাইসেমিয়ার লক্ষণ দেখা দিলে সঙ্গে সঙ্গে রক্তে গ্লুকোজ মাপতে হবে। হাইপোগ্লাইসেমিয়ার লক্ষণগুলো হচ্ছে দুর্বল লাগা, মাথা ঝিমঝিম করা, চোখে ঝাপসা দেখা, ঘাম হওয়া, হাত কাঁপা, মাথা শূন্য বোধ হওয়া ইত্যাদি।

চিকিৎসকের পরামর্শ নিন

* রক্তে গ্লুকোজের পরিমাণ ৩.৯ মিলিমোল বা ৭০ মিলিগ্রাম/ডেসিলিটারের কম হয়ে গেলে

* রক্তে গ্লুকোজের পরিমাণ ১৬.৬ মিলিমোল বা ৩০০ মিলিগ্রাম/ডেসিলিটারের বেশি পাওয়া গেলে

এবং যেকোনো আকস্মিক অসুস্থতায় চিকিৎসকের পরামর্শ নিন।

রমজানে ইনসুলিন ও ওষুধ

রক্ত পরীক্ষা করে আপনার চিকিৎসকের সঙ্গে রমজানে ওষুধ বা ইনসুলিনের ডোজ এবং শর্করা পরিমাপের সময়সূচি সম্পর্কে জেনে নিন। শারীরিক অবস্থা বিবেচনায় এই মাত্রা একেকজনের জন্য একেক রকম হতে পারে। সাধারণভাবে মেটফরমিন, ইনক্রিটিন গোত্রের ওষুধের মাত্রায় তেমন কোনো পরিবর্তন দরকার হয় না, কেবল সময়সূচি পরিবর্তন করলেই চলে। কিন্তু সালফোনাইল ইউরিয়া গোত্রের ওষুধ বা ইনসুলিনের বিষয়ে বিশেষজ্ঞ মতামত জরুরি। রমজান মাসে রক্তে গ্লুকোজের মাত্রা পরিমাপ করে কীভাবে ওষুধ বা ইনসুলিনের মাত্রা সমন্বয় করবেন, তা-ও শিখে নিন।

A new study in the American Journal of Hypertension, published by Oxford University Press, suggests that higher yogurt intake is associated with lower cardiovascular disease risk among hypertensive men and women.

High blood pressure is a major cardiovascular disease risk factor. Clinical trials have previously demonstrated beneficial effects of dairy consumption on cardiovascular health. Yogurt may independently be related to cardiovascular disease risk.

High blood pressure affects about one billion people worldwide but may also be a major cause of cardiovascular health problems. Higher dairy consumption has been associated with beneficial effects on cardiovascular disease-related comorbidities such as hypertension, type 2 diabetes, and insulin resistance.

For the current analyses, participants included over 55,000 women (ages 30-55) with high blood pressure from the Nurses' Health Study and 18,000 men (ages 40-75) who participated in the Health Professionals Follow-Up Study.

In the Nurses' Health Study, participants were asked to complete a mailed 61-item questionnaire in 1980 to report usual dietary intake in the preceding year. Participants subsequently reported any interim physician-diagnosed events including myocardial infarction, stroke, and revascularization. Permission was requested to access medical records to confirm all reported new diagnoses.

Higher intakes of yogurt were associated with a 30 percent reduction in risk of myocardial infarction among the Nurses' Health Study women and a 19 percent reduction in the Health Professionals Follow-Up Study men.

There were 3,300 and 2,148 total cardiovascular disease cases (myocardial infarction, stroke, and revascularization) in the Nurses' Health Study and the Health Professionals Follow-Up Study, respectively. Higher yogurt intake in women was associated with a 16 percent lower risk of undergoing revascularization.

In both groups, participants consuming more than two servings a week of yogurt had an approximately 20 percent lower risks of major coronary heart disease or stroke during the follow-up period. When revascularization was added to the total cardiovascular disease outcome variable, the risk estimates were reduced for both men and women, but remained significant.

Higher yogurt intake in combination with an overall heart-healthy diet was associated with greater reductions in cardiovascular disease risk among hypertensive men and women.

New research shows there might be health benefits to eating certain types of dark chocolate. Findings from two studies being presented today at the Experimental Biology 2018 annual meeting in San Diego show that consuming dark chocolate that has a high concentration of cacao (minimally 70% cacao, 30% organic cane sugar) has positive effects on stress levels, inflammation, mood, memory and immunity. While it is well known that cacao is a major source of flavonoids, this is the first time the effect has been studied in human subjects to determine how it can support cognitive, endocrine and cardiovascular health.

The flavonoids found in cacao are extremely potent antioxidants and anti-inflammatory agents, with known mechanisms beneficial for brain and cardiovascular health. The following results will be presented in live poster sessions during the Experimental Biology 2018 meeting:

Dark Chocolate (70% Cacao) Affects Human Gene Expression: Cacao Regulates Cellular Immune Response, Neural Signaling, and Sensory Perception

    This pilot feasibility experimental trial examined the impact of 70 percent cacao chocolate consumption on human immune and dendritic cell gene expression, with focus on pro- and anti-inflammatory cytokines. Study findings show cacao consumption up-regulates multiple intracellular signaling pathways involved in T-cell activation, cellular immune response and genes involved in neural signaling and sensory perception -- the latter potentially associated with the phenomena of brain hyperplasticity.

Dark Chocolate (70% Organic Cacao) Increases Acute and Chronic EEG Power Spectral Density (μv2) Response of Gamma Frequency (25-40Hz) for Brain Health: Enhancement of Neuroplasticity, Neural Synchrony, Cognitive Processing, Learning, Memory, Recall, and Mindfulness Meditation

    This study assessed the electroencephalography (EEG) response to consuming 48 g of dark chocolate (70% cacao) after an acute period of time (30 mins) and after a chronic period of time (120 mins), on modulating brain frequencies 0-40Hz, specifically beneficial gamma frequency (25-40Hz). Findings show that this superfood of 70 percent cacao enhances neuroplasticity for behavioral and brain health benefits.

Traditional Chinese medicine might be effective as a complement or alternative to traditional Western medicine for primary and secondary prevention of heart disease, according to a state of the art review paper published today in the Journal of the American College of Cardiology.

Heart disease is the number one cause of death worldwide, and despite advances in Western medicine for treating and preventing heart disease, unmet needs remain. As a result, traditional Chinese medicine is being increasingly looked at as a supplement to Western medicine, but to date randomized controlled trials are overall of poor quality and flawed.

Western scientists often reject Chinese medicine for specific reasons: the formula consists of dozens of ingredients with many chemical molecules, making it hard to clarify the therapeutic mechanism; the medications available in China do not undergo the same rigorous approval process as Western drugs to guarantee efficacy and safety; and most trials were conducted in China by traditional Chinese medicine physicians with medications largely unavailable in the United States.

Researchers in this review looked at studies published over the past 10 years on randomized controlled trials of traditional Chinese medicine used for patients with hypertension, dyslipidemia, diabetes/pre-diabetes, atherosclerotic cardiovascular disease and chronic heart failure to assess the efficacy and safety of traditional Chinese medicine.

In all, certain Chinese medications showed suggested benefits for each of the cardiovascular health conditions studied. For example, researchers looked at eight randomized controlled trials on traditional Chinese medicine and hypertension. The evidence indicated that Tiankuijiangya, Zhongfujiangya, Qiqilian, Jiangya and Jiangyabao have antihypertensive effects and a good safety profile, making them a potential good alternative for patient intolerant of or who cannot afford Western medications.

However, whether those benefits transferred into long-term positive cardiovascular outcomes would have to be determined by long-term trials.

"Of note, one should bear in mind that traditional Chinese medicine medications are usually prescribed as complex formulae, which are often further manipulated by the practitioner on a personalized basis," said Yuxia Zhao, senior author of the review and a physician in the Department of Traditional Chinese Medicine at Shandong University Qilu Hospital in Jinan, Shandong, China. "The pharmacological effects and the underlying mechanisms of some active ingredients of traditional Chinese medications have been elucidated. Thus, some medications might be used as a complementary and alternative approach for primary and secondary prevention of cardiovascular disease."
