How to reduce Unfair means in examination hall?

Every time we try to mitigate unfair means in examination hall. But in each examination time we get 1 or more student adopting it. Do you have any suggestion/idea to mitigate this?

Fahad Zamal:
We should include more invigilator in exam hall. Its really tough for us to cover a big hall for 1/2 people.
