Dry eye syndrome
also known as keratoconjunctivitis sicca (KCS), keratitis sicca, sicca syndrome, xerophthalmia, or simply dry eyes, is an eye disease in which tear film evaporation is high or tear production is low.According to the National Health Service (NHS), UK, approximately between 17% and 30% of people have dry eyes at some time in their life.


Dry eye syndrome is affecting an increasing number of people worldwide.
What are the signs and symptoms of dry eye syndrome?

A patient with dry eye syndrome may have the following signs and symptoms:

a. Stringy mucus in or around the eyes
b. Eye sensitivity to smoke
c. Eye sensitivity to wind (eyes water more in the wind)
d. Redness of the eyes
e. Eye fatigue, even after reading for a relatively short period
f. Photophobia - sensitivity to light
g. Discomfort when wearing contact lenses
h. Tearing


What are the causes of dry eye syndrome?

Reduced tear production is also linked to:

1. Inflammation
2. Radiation
3. Diabetes
4. Rheumatoid arthritis
5. Vitamin A deficiency
6. Refractive eye surgeries

The following may also cause dry eyes:

a. Sun
b. Wind
c. High altitude
d. A very dry climate
e. Hot blowing air
f. Very dry air (airplane cabin air)
g. Working in front of a computer monitor (may slow down blinking rate)
h. Driving a vehicle (may slow down blinking rate)
i. Reading (may slow down blinking rate)
j. Smoke
k. Contact lenses
l. HIV

Prevention of dry eye syndrome

Keep the eyes clean - people with clean eyes are less likely to develop blepharitis. Use warm/hot (not boiling) water, dab some cotton wool into it, and gently clean your eyes. Massage your eye gently with a clean finger in a circular motion - this will help move the mucus from the eyelid glands.

Wind - wear wraparound glasses for protection from wind and hot air, which can cause dry eyes.

Computer monitor - the monitor should be just below eye level.

Avoid smoky places - if you smoke, give up. If you don't, avoid places where people smoke.

Humidifier - a humidifier in the home will help moisten the air, which will help reduce the risk of dry eyes. Some people say that spraying curtains with a fine spray of water also helps keep the air humid.

Nutrition - evidence is starting to appear indicating that a diet rich in omega-3 fatty acids may help prevent dry eye syndrome. Sources of omega-3 fats include salmon, trout, tuna, herring and sardines. If you are vegetarian/vegan, the following foods are rich in omega-3 fatty acids: canola oil, English walnuts, flax oil, ground flax seed, hemp oil, hemp seed, olive oil, pumpkin seeds, and soybeans.

[Medical and other Uses]   Function:clearing away heatand toxic materials
Syndromes to be treated: affection due to wind and heat malaria, hyperactivity cough,heat stroke, fever, enteritis, eczema, herpes zoster, carbuncle sores poison.
 
 
[Dosage]   Oral administration:decoction, 10~30g;
Topicalapplication: in right amount, smashed and smear on the skin.

Chemical Constituents :
 
Leaves and stems contain n-alkanes, hentriacontane, octacosanoate, sterols including α-spinasterol, fatty acids, free alcohols, proteins and mixture of saponins, composed of oleanolic acid, D-glucose and D-glucoronic acid. It is a good source of calcium, also contains phosphorous, iron, nicotinin acid, ascorbic acid and protein. Roots contain α-spinasterol, octacosanoate and a number of saponins, β-sitosterol, stigmasterol, campesterol, cholesterol and stearic, oleic and linoleic acids, quercetin and rutin also isolated from the plant (Ghani, 2003; Rastogi & Mehrotra, 1993).

Uses:
Rheumatic pain, pain in the bones, blood or pus coming out with urine. The leaves and stems are cooked and eaten as vegetable as remedy for rheumatic pain and pain in the bones. Juice obtained from one crushed root is taken twice daily for 3 days as remedy for blood or pus coming out with urine. (Barisal) [Chowdhury, 2010]
Root paste is slightly warmed and applied on locally for boils. Whole plant is used in Increasing cow milk (Garo) [Anisuzzaman, 2007]
Slightly warm root paste is applied locally on boils. Whole plant paste is used as ointment for rheumatic pain. See also Benincasa hispida (Mandi) [Pavel, 2007]
Decoction of whole plant of Amaranthus spinosus is drunk twice daily to treat malarial fever. To stop bleeding from the nose and mouth, decoction of roots of both plants is drunk immediately (Chakma) [Rahman, 2007].
Fresh leaves are boiled and taken as vegetables (Khumi) [Uddin, 2010].

The plant is cooling, laxative, diuretic, stomachic, antipyretic and galactagogue; improves appetite; useful in burning sensation, hallucination, leprosy, piles, bronchitis, leucorrhoea, constipation and flatulence. Decoction of the herb is used as a mouth-wash for toothache. The root is heating and expectorant; lessens the menstual flow; useful in leucorrhoea, leprosy and eczema. It is considered specific in gonorrhoea. The boiled leaves and roots are given to children as a laxative; they are applied as an emollient poultice to abscesses, boils and burns. Root juice along with sugar or molasses is given in dysentery.

All severely malnourished children have vitamin and mineral deficiencies. As giving iron in acute phase can make infections worse, although anaemia is common, do NOT give iron initially but wait until the infection is controlled, child has a good appetite and starts gaining weight (usually by the second week)

Treatment

Give:

Vitamin A orally on Day 1 unless there is definite evidence that a dose has been given in the last month (for age >12 months, give
200,000 IU; for age 6-12 months, give 100,000 IU; for age 0-5 months, give 50,000 IU)

 Give daily for at least 2 weeks:


Multivitamin supplement (without iron)

Folic acid 1 mg/d (give 5 mg on Day 1)

Zinc 2 mg/kg/d

Copper 0.3 mg/kg/d (if available)

Elemental iron 3 mg/kg/d but only when gaining weight (start in rehabilitation phase when gaining weight)
